The Care Finder program provides seniors with one-on-one support to understand the aged care system, access funding and connect with service providers.
We know navigating aged care can feel overwhelming for many individuals and families and we are here to help you on the journey so you can find the support that best meets your unique needs.
Who we help?
People who require intensive support to navigate the My Aged Care System
- People over 65
- People from multicultural backgrounds
- Aboriginal and Torres Strait Islander peoples over 50
- Low income earners, homeless, or at risk of homelessness over 50

Specialist and intensive support as a CALD provider, MCCGC’s service model is based on providing all clients across our programs specialised and intensive support that is customised to each individual’s unique needs. We work with seniors, people with disability and community members from a wide variety of cultural backgrounds and understand the extra time and effort involved in serving people who face additional barriers to access including social isolation, low literacy levels and cognitive impairment.
Our Care Finder model takes into consideration each client’s personal circumstances, preferences, and needs and works with them to overcome barriers and become comfortable and confident interacting with the aged care system and other community supports. We use language support as needed, with bilingual staff that speaks priority languages and supplement with language support workers and interpreters when necessary. We also have experienced staff who can provide additional support including our Clinical Care Manager, Social Worker, and Dementia Respite Staff.
